Church Conference Forms
You can complete & submit a form in one session, or leave the form to complete & submit later. To save your form in progress click on the Save and Continue button on the right side. A link will appear at the top of the page. *It is important to highlight, copy and save this link before exiting out. You may paste this link in the address bar of your browser to re-enter your session at any time. Note: once you submit the form, the link will no longer be available. *
When attaching files in the forms, Word Docs and PDFs are accepted. (No .jpgs or .pngs) Please write headings at the top of attached forms that includes: Name of the Report (include year), Pastor’s Name, Church Name (City: Church Name).
We recommend printing a copy of each completed form to retain for your church records. Click on the “print button” at the top before hitting submit.
We recommend using a browser other than Internet Explorer such as Google Chrome or Firefox to be able to print.
To start a fresh form, click on the Northern Illinois Conference logo at the top of the page.
To print a blank form to share with your leadership, click on the tabs below to open the desired form and click the "print button" at the top.
*indicates a required field (if this field is not filled in, an error message will appear after hitting submit and required fields will appear in red)
